This annual declaration form is
required for the purposes of determining your membership
subscription payment and also for collating industry statistics as
shown in the BMF’s Key Performance Indicators report. Accurate
statistics are vital to our function as the national representative
body for the industry in the UK.
All information is treated in
total confidence. Please note that in accordance with BMF
rules, incorporating the Code of Practice, without a completed
declaration form your company will be automatically moved into the
next payment band resulting in a higher subscription fee. We do not
want to charge you more than is necessary so please return this
form.
Please ensure that you complete and submit this form to
the BMF by 20 April 2012.
